- High-dose omega-3 supplements were not associated with improved memory or cognitive function in cognitively unimpaired older adults at high risk for dementia, a new clinical trial suggested.
- Researchers found evidence of the omega-3 docosahexaenoic acid (DHA) in cerebrospinal fluid (CSF) — indicating the nutrient reached the brain — but it didn’t translate to benefits in hippocampal volume or cognitive performance.
- At 6 months, the intervention group showed a 17% increase in CSF DHA levels, indicating brain penetrance, whereas the placebo group reported a decrease (mean change for DHA, 0.17 vs -0.02; P < .0001).
- The authors noted, “The failure of high-dose DHA supplementation to modify clinical outcomes despite confirmed CNS delivery suggests that membrane phospholipid composition changes alone are insufficient.”
- Adverse events occurred at about the same rate in both groups (40.3% in the DHA group vs 40.9% in the placebo group).
- Serious adverse events occurred in 2.8% of DHA participants vs 7.2% of the placebo groups.
- Spinal fluid drawn from older adults swallowing two grams of DHA a day showed the omega-3 piling up exactly where it was meant to go: inside the brain, lifting concentrations by roughly 17 per cent.
- The brain took the bait. The disease carried on regardless.
- Researchers suspect that inflammation, driven by other health problems, may break the fatty acids down before they can do any good, and that an enzyme called cPLA2 plays a role.

A clinical trial involving older adults at high risk for dementia revealed that high-dose omega-3 supplements, particularly DHA, failed to improve memory or cognitive function.1
After 24 months, participants taking DHA showed no significant differences in memory scores or hippocampal volume compared to those on a placebo.2

Spinal fluid analysis confirmed a 17% increase in DHA levels, indicating successful brain penetration, yet cognitive performance remained unchanged.37
Lead investigator Hussein N. Yassine, MD, stated, “While omega-3s play an important role in forming brain cell connections needed for cognition, our results do not support supplements as a preventive measure against Alzheimer’s.”
The study also noted that inflammation from other health issues might hinder the effectiveness of DHA, as it was broken down before providing any benefits.9
Adverse events were similar in both groups, with 40.3% in the DHA group and 40.9% in the placebo group experiencing complications.5
The findings challenge previous assumptions about the cognitive benefits of omega-3 supplementation, emphasizing that simply increasing DHA levels in the brain is insufficient for improving cognitive health.
