The Directorate General of Civil Aviation (DGCA) has launched a probe into Air India following the Boeing 787-8 Dreamliner crash, uncovering "systemic failures" and "multiple violations" in crew scheduling protocols.

The regulator has ordered the removal of three senior officials directly accountable for lapses in licensing, rest, and recency requirements. These officials include Choorah Singh, Divisional Vice President; Pinky Mittal, Chief Manager in Crew Scheduling; and Payal Arora, Crew Scheduling – Planning.

"The DGCA on Saturday (June 21, 2025) directed Air India to remove three officials, including a top executive part of flight operations department, from their roles and responsibilities for 'systemic failures' and 'multiple violations' in flight crew scheduling," the regulator stated.

The findings emerged during a review of Air India's transition between crew scheduling software systems, revealing lapses that compromised safety standards.

Additionally, the DGCA issued a showcause notice to Air India for violating Flight Duty Time Limitations (FDTL) norms on two Bangalore-London flights in May.

In response, Air India acknowledged the directive and confirmed implementation, appointing its Chief Operations Officer to oversee the Integrated Operations Control Centre (IOCC) to ensure compliance.

"We acknowledge the regulator's directive and have implemented the order. In the interim, the company's Chief Operations Officer will provide direct oversight to the Integrated Operations Control Centre (IOCC)," the airline said.

This regulatory action comes as Air India, now under Tata Group ownership since 2022, strives to rebuild its reputation amid ongoing scrutiny following the Dreamliner crash and past criticisms over service quality.
